of your primary care through this organization.  
You agree to call our clinic for your medical needs.  
You agree to utilize our After Hours Clinics if an appointment is not available at our location.
You agree not to utilize walk-in clinics.
If you fail to meet your obligations as a patient you may be de-rostered.
