Dear Patient,
As your family doctors, we are committed to assisting you with all your healthcare needs. The Ontario Health Insurance Plan (OHIP) covers most medical visits to our office; however, many of the additional services we provide are not insured by OHIP.
These uninsured services include faxed prescription renewals, as well as the completion and transmission of forms and notes. In the past, many patients who utilized these services chose to pay for them through an all-inclusive annual fee.
You may opt to pay for these services either via an all-inclusive annual fee or individually as rendered, according to the schedule of services and fees provided below. Please note that participation in the annual subscription is completely voluntary and will not affect the quality or continuity of your care.
We encourage you to consider the single-payment yearly option, as it reduces administrative burden and allows us to dedicate more time to patient care.
PLEASE AVOID THE USE OF WALK-IN AND VIRTUAL CLINICS
When you attend a walk-in clinic, your diagnosis, treatment, and test results are not forwarded to us. Additionally, each visit to a walk-in clinic results in a financial penalty for your physician.
This is not the case when you attend our After-Hours Clinic.
Duplication of services—such as visiting a walk-in clinic and then consulting your physician for the same issue—places additional strain on an already overburdened healthcare system.
After-Hours Clinic InformationDowntown Toronto Doctors is part of a Family Health Organization (FHO), which operates an After-Hours Clinic. This clinic is available to all rostered patients during the following hours:
- Monday to Friday: 5:00–8:00 PM
- Saturday: Call for availability
- You will receive a scheduled appointment time.
- A report of your visit will be sent to us, allowing for proper follow-up and further treatment as necessary.
Block Fee Rates

Included Under Block Fee
- Allied health notes (e.g., physiotherapy, massage, orthotics, sick notes): $30
- Fax or phone prescription renewals: $25 (at physician's discretion)
- Lost prescriptions, requisitions, or referrals: $30
- Disability Tax Credit form: $75
- Disability Parking Permit form: $30
- Medical records prepared on CD or via email: $50
- Travel advice or consultation: $30
- Travel vaccine administration: $15
- Early pap smear (performed earlier than the recommended 3-year screening): $55 (+ additional lab fee charged directly by the lab)
- Requisitions and referrals initiated by the patient: $50 (at physician’s discretion)
- Any medical form that can be completed within a 15-minute appointment will incur a $50 fee. To save time and reduce costs, please fill out as much of the form as possible before your appointment. Forms requiring more than 15 minutes to complete will be charged based on OMA rates and are NOT covered under the block fee.
